Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It collects and distributes cleaned trial data, ratings and evaluations using PRECIS-2. This permits a variety of meta-epidemiological analyses to examine the effect of treatment across trials with different levels of pragmatism.
Background
Pragmatic trials provide real-world evidence that can be used to make clinical decisions. However, the use of the term "pragmatic" is inconsistent and its definition and assessment requires clarification. Pragmatic trials should be designed to guide clinical practice and policy decisions, not to confirm a physiological or clinical hypothesis. A pragmatic study should strive to be as close to real-world clinical practice as possible, such as its selection of participants, setting up and design, the delivery and execution of the intervention, as well as the determination and analysis of outcomes and primary analysis. This is a significant difference between explanation-based trials, as described by Schwartz & Lellouch1 which are designed to prove a hypothesis in a more thorough manner.
Truely pragmatic trials should not conceal participants or the clinicians. This could lead to a bias in the estimates of treatment effects. Methods

Furthermore, a common feature of pragmatic trials is that researchers attempt to make their findings more valuable by studying subgroups of the sample. In the case of the pragmatic trials included in this meta-analysis this was a significant problem since the secondary outcomes were not adjusted to account for differences in the baseline covariates.
Additionally practical trials can present challenges in the collection and interpretation of safety data. This is because adverse events are usually self-reported and are prone to reporting delays, inaccuracies or coding deviations. Therefore, it is crucial to improve the quality of outcome for these trials, ideally by using national registry databases instead of relying on participants to report adverse events in the trial's database.

While the definition of pragmatism may not require that all trials are 100 percent pragmatic, there are some advantages to incorporating pragmatic components into clinical trials. These include:

They found that pragmatic reviews scored higher on average across all domains, however they scored lower in the primary analysis domain.
This distinction in the primary analysis domains could be explained by the way most pragmatic trials approach data. Some explanatory trials, however, do not. The overall score for pragmatic systematic reviews was lower when the domains of organisation, flexible delivery and following-up were combined.
